Step 1 - Filling out the form

Personal information: Fill out all personal
information as requested. Your copy of the form will be returned to you
in a window envelope using your written name and address, so be certain
this information is complete and legible.
- Course I wish to take (prerequisite/requirement I wish to challenge):
Fill in the Chabot course and course number of the course you wish
to take and prerequisite course as directed.
- Reasons (A - E):
- Reason A: Check here if you believe that you have met
the course equivalent of the prerequisite at another college. Supply the
name of college and term of your attendance there and course name and
number of Chabot’s prerequisite. To use this reason, you must provide
documentation to verify both course equivalence and successful completion
of the course (a grade of C or higher). See Providing Documentation below
for instructions.
- Reason B: Check here if you believe you have pertinent
knowledge and/or experience that would fulfill the prerequisite requirement.
Please provide an explanation on a separate piece of paper.
- Reason C, D, E: Check here if appropriate. Please provide
an explanation on the space provided and/or on a separate piece of paper.
- Sign and date the form.
Step 2 - Providing Documentation
For English & ESL Classes:
Students are responsible for establishing that they have the skills to challenge
the prerequisite. Students may choose to submit one or more of the following:
- Option A: Sample papers from an English class or a class
that required substantial writing. These must be student’s own work.
- Option B: Work-related documents for which she/he is the
only author supplemented by writing they have done in a class.
- Option C: The course description and their writing assignments
from a similar course taken at another institution.
- Option D: Transcript (evaluated by an accredited agency
if necessary) accompanied by their own sample writing assignments from classes
on the transcript. This step is required for all students
challenging ESL.
For all other classes:
Attach required documentation from one of the following options to verify
course equivalent:

Option A: A course outline or syllabus.
- Option B: A school catalog description if it describes
specific course content.
- Option C: A letter from the instructor of the course outlining
the course content.
and
Attach required documentation from one of the following options
to verify a grade of “C” or higher:

Option A: Academic Transcripts. International
transcripts/credentials must be certified by an International evaluation
service.

Option B: Report card containing the proposed
equivalent course.

Option C: Original letter from the course
instructor on school letterhead, indicating the completion of the course
with a grade of “C” or higher.
Step 3 - Submitting the form & documentation to the appropriate Division
