This is an application to have your enrollment fees waived. This fee waiver is for California residents only. If you need money to help with books, supplies, food, rent, transportation and other costs, please complete a Free Application For Federal Student Aid (FAFSA) immediately at www.fafsa.gov.

Lifetime Pell Eligibility Acknowledgement

*New! Pell Lifetime Eligibility Used (LEU) Federal regulations now limit students to a lifetime maximum Pell Grant of 12 semesters / six years or full time equivalent (600%) at any community college, vocational school or four year/bachelor program. Once you reach the 600% Pell LEU, you will be ineligible for any additional Pell Grants beginning in 2012-13 school year. Log into National Student Loan Data System (NSLDS)your PIN and view your LEU, on the Financial Aid Review page.

What this means for you: You must plan now for your future, especially if you plan to transfer to a four year college to pursue a Bachelors degree. The six year limit is a LIFETIME limit, regardless of completion of your academic goals or Bachelors degree. This regulation is in effect even if you have had an appeal approved after failure to meet academic progress, it supersedes any prior offer of aid/award, and the Department of Education allows for no appeal or exception. If you do not wish to receive any additional Pell Grant at Chabot, complete the Lifetime Pell Eligibility Acknowledgement form.

Identity Verification

 The student must appear in person at the Financial Aid Office to verify his or her identity by presenting a valid government issued photo identification (ID), such as, but not limited to, a driver’s license, other state issued ID, or passport.  The institution must maintain a copy of the student’s photo ID that is annotated with the date it was received and the name of the financial aid specialist authorized to collect the student’s ID.

Agency Certification & Untaxed Income

Submit to your case worker or the Social Security Office for completion and return our office. If using this form to establish eligibility for BOG A, you must submit it with a completed BOG application.

Parent Update Form

You must provide the last name, first initial, and social security number for each parent or stepparent whose information is provided on your Financial Aid application. Failure to provide this information will result in a rejected financial aid application, and we cannot proceed with file review without it. Please use this form to update your parents information by completing all items as instructed, and have each parent sign and date form.

Verification of Childcare Expenses

Please provide the requested information below on your dependents 12 years of age or under, whom you included in question 96 on your FAFSA (or in Section 4 of FAFSA on the Web), number of family members, who will be in childcare during the Fall 09/ Spring 10 semester(s). Do not include private school tuition, including kindergarten.  Childcare expenses may be included in your cost of attendance for the year; however, they do not represent any implied financial aid award to cover these expenses.

Verification of Disability Expenses

Disability related expenses may be included in your cost of attendance for the year, however, there is no implied financial aid award to cover these expenses. If you have costs associated with a disability, please complete this form to verify the total amount of the expenses, and how much of these expenses are paid for by an outside funding agency, or reimbursed to you.

SNAP Food Stamp Worksheet

On your Free Application for Federal Student Aid (FAFSA) or your Verification Worksheet, you indicated that you or member(s) of your household received Food Stamps or participated in the Supplemental Nutrition Assistance Program (SNAP) during 2011 and/or 2012. As part of the verification process, you must confirm and provide proof of this benefit. Please complete the information below, and if you are or did receive benefits please provide the appropriate documentation. Once completed please return all information to the Financial Aid Office.

Child Support Paid Verification

On your Free Application for Federal Student Aid (FAFSA) or your Verification Worksheet, you indicated that you, your spouse (if married) or your parent(s) (if dependent), paid child support during the year 2012. As part of the verification process, you must confirm the amount of child support paid, by whom the child support was paid, and to whom it was paid.

Appeal for Reinstatement or Extension of Financial Aid

If a student has extenuating reasons for failing to meet satisfactory academic progress standards (over 90 attempted units, or unsatisfactory performance), s/he may complete and submit to the Financial Aid Office a Petition for Reinstatement or Continuation of Financial Aid.

Consortium Agreement

A consortium agreement allows Chabot to take into account units a student takes at another college that are REQUIRED for Chabot degree for financial aid purposes. This is a courtesy only, will be considered only through the first seven weeks of the semester, and are approved on a case by case basis.  All decisions by the FAO are final.  Only complete if you will be enrolled less than full-time at Chabot AND enrolled at another college for the same term you will be attending Chabot. Grades from other schools are required prior to any subsequent disbursement of aid at Chabot.

Release of Information FERPA Form

In compliance with FERPA (Family Educational Rights and Privacy Act) laws, and to protect the privacy of our students, the Chabot College Financial Aid Office will not discuss a student's financial aid file or eligibility with anyone other than the student. If a student wishes to authorize the Financial Aid Office to release information to or discuss financial aid with another person (parent, spouse, advocate, etc.), the student may complete the Release of Information Form. The release is good for one academic year only.

The release form must be signed by the student in person at the Financial Aid Office, or must be officially notarized. Photo ID is required of both student and representative at each visit.

Cal Grant GPA Verification Form

Cal Grant deadline for community college students is September 2nd. Students must have completed at least 24 college units, completed a FAFSA application, Cal Verification Form, and have a minimum 2.0 GPA. Students must complete the form and submitted to the California Student Aid Commission, visit www.calgrants.org for more information.

Students who need to verify their identity for the Higher One account, should complete the CIP form and bring appropriate identification verification information requested.

Change of Personal Data Form

Students should complete a Request to Change Personal Data to make changes to any or all of the following:

  • Name
  • Address
  • Telephone number
  • Intended major
  • Social security number
  • Home Campus Code
  • E-mail address

Completed forms should be submitted to the Office of Admissions & Records.

Independency Status Change Request (come to our office)

A student who wishes to request a dependency status change must meet with a Financial Aid advisor. Students 24 years or older are automatically considered independent. You must be able to document the reasons you present in your request. In some cases, approval of this request can decrease your eligibility for federal aid.

Special Circumstances Petition  (come to our office)

Please see the Financial Aid Office staff if you wish an additional review of your eligibility based on the decrease in family income or other special circumstances after filing the FAFSA.
