Financial Aid Office2009-2010 Financial Aid Forms
Students must use the 2009-2010 FAFSA or BOG application to waive enrollment fees for the Summer 2009, Fall 2009, and Spring 2010. The 2009-2010 FAFSA will not waive your summer 2010 enrollment fees, but must be used for consideration of other forms of financial aid, such as the Pell Grant.
This document is required of all financial aid applicants each year. Please complete all items, sign and date, and turn in to our office with all other requested verification documents. Submit to your case worker or the Social Security Office for completion and return our office. If using this form to establish eligibility for BOG A, you must submit with a completed BOG application. You may have left question 23 of the FAFSA blank, or indicated you have a drug related conviction which may affect your eligibility for financial aid. Failure to provide this information will result in a rejected financial aid application, and we cannot proceed with file review without it. Please use this form to update your parents information by completing all items as instructed, and have each parent sign and date form. Please provide the requested information below on your dependents 12 years of age or under, whom you included in question 96 on your FAFSA (or in Section 4 of FAFSA on the Web), number of family members, who will be in childcare during the Fall 09/ Spring 10 semester(s). Do not include private school tuition, including kindergarten. Childcare expenses may be included in your cost of attendance for the year; however, they do not represent any implied financial aid award to cover these expenses. Disability related expenses may be included in your cost of attendance for the year, however, there is no implied financial aid award to cover these expenses. If you have costs associated with a disability, please complete this form to verify the total amount of the expenses, and how much of these expenses are paid for by an outside funding agency, or reimbursed to you.

If there are questions about untaxed income, or we need your certification that you are not required to file federal taxes, you may be required to complete this form.
Academic Competitiveness Grant (ACG) Rigorous Course of Study Validation Form. This form helps validate the eligibility of high students for an additional federal grant. When validation is complete the form is returned to the Chabot Financial Aid Office. Please read enclosed information regarding the ACG program.
If a student has extenuating reasons for failing to meet satisfactory academic progress standards (over 90 attempted units, or unsatisfactory performance), s/he may complete and submit to the Financial Aid Office a Petition for Reinstatement or Continuation of Financial Aid.
You indicated on your FAFSA that you will receive veterans education benefits. Complete this form to verify the type and amount of the VA benefit you receive. If you are unsure please contact the Veteran's Services office. Update: No longer required for Federal Financial Aid but still required for Cal Grant Awards. A student who wishes to request a dependency status change must meet with a Financial Aid advisor. Students 24 years or older are automatically considered independent. You must be able to document the reasons you present in your request. In some cases, approval of this request can decrease your eligibility for federal aid. Please see the Financial Aid Office staff if you wish an additional review of your eligibility based on the decrease in family income or other special circumstances after filing the FAFSA.
