The 2009 application is open as of October 15, 2008 for the class beginning in August 2009. Sixty nursing students are admitted each August to the two year program leading to the AA degree in Nursing. All applications must be post marked on or before January 15, 2009.
Step 1: A Chabot College Application for Admission must be submitted to the college prior to applying to the Nursing Program. This application can be submitted electronically by going to http://www.cccapply.org/Applications/California_Community_College/apply/Chabot_College.html and is available year round.
Step 2: The Nursing Program Application in(PDF) (Application in WORD) is only available online from October 15, 2008 through January 15, 2009 at 5 PM. Click the word Application(PDF) or Applicaiton in WORD above to open the file. 'Tab' through the form to complete online.
Application Note: If you are unable to open application in Adobe Reader or in Word please email nurse@chabotcollege.edu. Indicate your name and address, a hard copy will be mailed to you.
Step 3: Print, sign and MAIL the nursing program application to Admission and Records Office, Chabot College, 25555 Hesperian Blvd. Hayward, CA 94545 with a postal mark no later than January 15, 2009. If you are currently taking or registered to take one or more of your pre-requisite natural science courses, verification of enrollment is required with the application. It is recommended you obtain a "Certificate of Mailing" and "Return receipt" from the U.S. Postal Service at the time of mailing.
Step 4: The student must request official transcripts from all previously attended schools and colleges to be sent from those institutions directly to general Admissions and Records no later than January 31, 2009. If you have taken college classes while attending high school those transcripts must also be included. Check Class-Web for the status of transcripts received online under your student information. It is your responsibility to check the status of receipt of your transcript(s). No one will contact you about missing transcripts and you will be disqualified if all transcripts have not been received prior to 01/31/2009.
NOTE: Failure to follow all the above steps will result in the application being removed from eligibility.
Please note: Students who have completed two of the three prerequisite science courses before December 31, 2008 may submit an application by January 15, 2009. However, the following stipulations are in effect:
a. If you have just completed any pre-requisites in the 2008 FALL semester you must provide a letter from the instructor with your current grade with the application. By January 31, 2009 you must provide an updated transcript for ANY classes taken in Fall 2008. Your overall GPA must be calculated, current transcripts are required.
b. Evidence of current enrollment in the third prerequisite science course must be
submitted with the application;
c. The course must be verified as having been completed by the
end of Spring Semester with a grade of "C" or higher and must been the grade
criteria for acceptance into the Nursing Program as outlined in MINIMUM
REQUIREMENTS, above.
c. The application will be entered into the pool of other
applicants and be subject to the selection process as outlined below, with the
understanding that failure to meet the criteria as outlines in item c.
will result in the applicant's name being withdrawn from the list of
those qualified for admission to the Nursing Program. No exception to this
rule will be made.
PROCEDURES FOR SELECTION INTO THE APPLICANT POOL AND ADMISSION TO THE NURSING PROGRAM
After the January 15, 2009 deadline, students who meet the minimum requirements of having completed the science prerequisites AND whose transcripts from all schools attended are on file will have their transcripts evaluated for the following:
a. grade received in English 1A or its equivalent;
b. Anatomy course taken once or repeated;
c. overall lower division grade point average
A criterion score (calculated from a., b., c. above) has been established for purposes of selection. Those meeting or exceeding the criterion score will be entered into the "candidate for admission" pool and chosen by a random selection method.
Qualified students will be required to take the TEAS (Test of Essential Academic Skills) through Assessment Testing Incorporated (ATI). Everye student must score 67 percent in their 'Adjusted Individual Total Score'. (*These levels maybe adjusted on or before the testing period.) Qualified students will be notified by mail of date, time and locations of tests after January 31, 2009.
The "candidate for admission" pool will be divided into
a. Verified residents of the geographical area served by the Chabot-Las Positas Community College District
b. Those residing outside of the area served by the District.
The first 60 applicants will be randomly selected from the District two resident pools for entry into the Nursing Program. Pool 1: Eligible residents of Castro Valley, Hayward, San Leandro, San Lorenzo, and Union City will be placed in the Chabot College location pool where 40 will be randomly selected. Pool 2: Eligible residents of Dublin, Livermore, Pleasanton, Sunol will be placed in the Extended Campus at the Valley Memorial Hospital location pool where 20 will be randomly selected.
Non-selected, but eligible residents of both pools will be entered into the "alternate" pool and randomly selected on rank-order basis and given an alternate number. The alternates will be subject to placement in either the Chabot College location or the Extended Campus location, depending on the slot vacated.
Those names selected in the first group of 60 will be notified no lather than April. Alternates will also be notified of their alternate status by mail no later than May 30.
