If a student wishes to authorize the Financial Aid Office to release information to or discuss financial aid with another person (parent, spouse, advocate, etc.), the student may complete the Release of Information Form. The release is good for one academic year only.

The release form must be signed by the student in person at the Financial Aid Office, or must be officially notarized. Photo ID is required of both student and representative at each visit.

Ability To Benefit Certification Form (PDF)

For students who do not have a high school diploma or equivalent and would like to qualify for financial aid, we provide a test to determine if the student meets the criteria to be eligible for aid. Students are usually referred by the Financial Aid Department for this test. This is a pass or fail test and is done by appointment only.  Please contact the Assessment Center to make an appointment.

Consortium Agreement (PDF)
