 Only print and submit required forms listed on your Zone or Class Web account.  Make sure you are choosing forms for correct financial aid/academic year. 

Please note: To access Financial Aid documents from this site, you need Adobe Acrobat Reader and a printer.  These are not PDF-fillable forms - do not try to convert to or open with Microsoft Word or other program.  You must print and complete the old fashioned way.  We hope to have PDF-fillable forms in the near future. 

Board of Governor's Fee Waiver Application - the Online Application is now available!  However, we encourage each student to complete a FAFSA to apply for the BOG Fee Waiver, as it is easier to qualify for BOG and potentially for additional funds for school. 
NOTE: If you have already completed a FAFSA application, please DO NOT complete a BOG waiver online.  Follow up with the FAO or check the Zone for your current application status.

 

If you are required to verify your Federal Tax Return information, please request a complete transcript of prior tax returns by calling the IRS at 1-800-829-1040 or at www.irs.gov .

Federal Verification Worksheet - Independent / Dependent

Untaxed Income Certification Form - Independent / Dependent

Certain applicants will be required to verify the untaxed income information they provided on the FAFSA.  If the Financial Aid Office determines that the amounts provided and/or verified do not appear to provide sufficient financial support for the number of household family members reported, the applicant and spouse/parents must explain how the family was financially supported during the 2014 calendar year.

If you were selected for this verification, if there are questions about untaxed income, or we need your certification that you are not required to file federal taxes, you will be required to complete this form. You may be required to provide proof from the IRS that you were not required to file federal taxes.

SNAP Food Stamp Worksheet

As part of the verification process, you must confirm and provide proof that you or member(s) of your household received Food Stamps or participated in the Supplemental Nutrition Assistance Program (SNAP) during 2013 and/or 2014.  If you are or did receive benefits please provide the appropriate documentation.

Child Support Paid Verification

On your Free Application for Federal Student Aid (FAFSA) or your Verification Worksheet, you indicated that you, your spouse (if married) or your parent(s) (if dependent), paid child support during the year 2014. As part of the verification process, you must confirm the amount of child support paid, by whom and to whom the child support was paid.

High School Verification Worksheet

On your Free Application for Federal Student Aid (FAFSA) you indicated your high school completion status. You have been selected for verification of this information.  If you did or will complete high school or a GED before the start of 2016-17 school year, you must confirm and provide any documentation requested (please see back page).

Identity Verification

The student must appear in person at the Financial Aid Office to verify his or her identity by presenting a valid government issued photo identification (driver’s license, other state issued ID, military ID, or passport). The institution must maintain a copy of the student’s photo ID, annotated with the date it was received and the name of the financial aid specialist authorized to collect the student’s ID.

Student Update Form

Use this form to update our office on your name, date of birth, social security number, gender, marital status, etc.  Failure to provide this information may result in a rejected financial aid application, and we cannot proceed with file review without it.

Parent Update Form

You must provide the last name, first initial, and social security number for each parent or stepparent whose information is provided on your Financial Aid application. Failure to provide this information will result in a rejected financial aid application, and we cannot proceed with file review without it. Please use this form to update your parents information by completing all items as instructed, and have each parent sign and date form.

Lifetime Pell Eligibility Acknowledgement

Pell Lifetime Eligibility Used (LEU) Effective July 1, 2012, students are limited to a lifetime maximum Pell Grant of 12 full time semesters / six years (600%) at any community college, vocational school or four year/bachelor program.  Once you reach the 600% Pell LEU, you are ineligible for any additional Pell Grant.  Log onto www.nslds.ed.gov  using your FSA ID and view your LEU, found on the Financial Aid Review page.  For more information:  http://studentaid.ed.gov/PORTALSWebApp/students/english/PellLimit.jsp 

What this means for you:  You must plan now for your future, especially if you plan to transfer to a four year college for a Bachelors degree.  The six year limit is a LIFETIME limit, regardless of completion of your academic goals or Bachelors degree. This regulation is in effect even if you have had an appeal approved for failure to meet academic progress, it supersedes any prior offer of aid/award, and the Department of Education allows for no appeal or exception.

Unusual Enrollment History (UEH) Notification

Effective 2013-2014, the U.S. Department of Education established new regulations to prevent fraud and abuse in the Federal Pell Grant Program by identifying students with Unusual Enrollment Histories (UEH).

If you received the Federal Pell Grant at multiple institutions in recent academic years
(2011-12, 2012-13, 2013-14, & 2014-15), your 2015-16 FAFSA was flagged for unusual enrollment history (UEH)
by the U.S. Department of Education.
 

All students with a UEH flag will be required to provide Chabot College with official academic transcripts
from all colleges and universities attended during the review period
,
as reported on NSLDS (National Student Loan Data System – (www.nslds.ed.gov
)

Release of Information FERPA Form

In compliance with FERPA (Family Educational Rights and Privacy Act) laws, and to protect the privacy of our students, the Chabot College Financial Aid Office will not discuss a student's financial aid file or eligibility with anyone other than the student. If a student wishes to authorize the Financial Aid Office to release information to or discuss financial aid with another person (parent, spouse, advocate, etc.), the student may complete the Release of Information Form. The release is good for one academic year only.

The release form must be signed by the student in person at the Financial Aid Office, or be officially notarized. Photo ID is required of both student and representative at each visit.

Agency Certification & Untaxed Income

Submit to your case worker or the Social Security Office for completion and return to our office. If using this form to establish eligibility for BOG A, you must submit it with a completed BOG application.

Verification of Childcare Expenses

Please provide the requested information below on your dependents 12 years of age or under, whom you included in on your FAFSA in number of family members, who will be in childcare during the Fall / Spring semester(s). Do not include private school tuition, including kindergarten. 

Childcare expenses may be included in your cost of attendance for the year; however, they do not represent any implied financial aid award to cover these expenses.

Verification of Disability Expenses

Disability related expenses may be included in your cost of attendance for the year, however, there is no implied financial aid award to cover these expenses. If you have costs associated with a disability, please complete this form to verify the total amount of the expenses, and how much of the expenses are paid for by an outside funding agency, or reimbursed to you.

Appeal for Reinstatement or Extension of Financial Aid

If a student has extenuating reasons for failing to meet satisfactory academic progress standards (over 90 attempted units, or unsatisfactory performance), s/he may complete and submit to the Financial Aid Office a Petition for Reinstatement or Continuation of Financial Aid with all required documentation.

Consortium Agreement

A consortium agreement allows Chabot to take into account units a student takes at another college that are REQUIRED for Chabot degree for financial aid purposes. This is a courtesy only, will be considered only through the first weeks of the semester, and are approved on a case by case basis.  All decisions by the FAO are final.  Only complete if you will be enrolled less than full-time at Chabot AND enrolled at another college for the same term you will be attending Chabot. Grades from other schools are required prior to any subsequent disbursement of aid at Chabot.

Board of Governor's (BOG Fee Waiver) Application

2016-2017 BOG Fee Waiver (Sum 2016, Fall 2016, Spring 2017) BOG Fee Waiver - Don't wait in line. Submit your application online today.

This is an application to have your enrollment fees waived. This fee waiver is for California residents only. If you need money to help with books, supplies, food, rent, transportation and other costs, please complete a Free Application For Federal Student Aid (FAFSA) immediately at www.fafsa.gov. BOG applications are only available online.

NOTE: If you have already completed a 2016-17 FAFSA, please DO NOT complete a BOG waiver online. Follow up with the Financial Aid Office or check the Zone for your current application status.

Verification of Family Size and Number in College

If there are questions about the number of members in your household and/or those counted in college, you may be required to complete this form. You may be required to verify this information, especially if there appears to be a significant difference from prior years.

Cal Grant GPA Verification Form

Cal Grant deadline for community college students is September 2nd. Students must have completed at least 24 college units, completed a FAFSA application, Cal Grant Verification Form, and have a minimum 2.0 GPA. Students must complete the form and submit to the California Student Aid Commission. Visit www.calgrants.org for more information.

Change of Personal Data Form

Students should complete a Request to Change Personal Data
to make changes to any of the following:

  • Name
  • Address
  • Telephone number
  • Intended major
  • Social security number
  • Home Campus Code
  • E-mail address

Completed forms must be submitted to the Office of Admissions & Records.

Independency Status Change Request (Must obtain in person from FAO)

A student who wishes to request a dependency status change must meet with a Financial Aid advisor. Students 24 years or older are automatically considered independent. You must be able to document the reasons you present in your request. In some cases, approval of this request can decrease your eligibility for federal aid.

Special Circumstances Petition (Must obtain in person from FAO)

Please see the Financial Aid Office staff if you wish an additional review of your eligibility based on the decrease in family income or other special circumstances that may affect financial eligibility after filing the FAFSA.
