Services. Waiver form is available at the health center.
- Present a valid Chabot Student picture ID with current sticker
or a copy of current class schedule to the health service personnel.

as your prepaid student health fee covers many services. However,
your student health fee does not cover hospital care, referrals to
specialists, dental, vision, or emergency transportation. For these
reasons, we strongly recommend you obtain supplemental health
insurance.
Student Health Services does not file for any
medical insurance reimbursement.
