The treatment center is located in room 2902. Usual hours of operation are 10:30 am - 6:30 pm on weekdays.


Treatment Center Rules:

  1. Dress appropriately
  2. No food, drinks or tobacco products
  3. Be respectful of training staff and other athletes
  4. No treatment during practice times

Athlete's Responsibilities

Before athletic participation, all athletes are to have the following completed and given to the athletic trainer:

  1. Completed physical exam performed by an MD, using the standard form.
  2. Athlete Information Form, stating emergency contact and insurance information.
  3. Any other academic/ administrative paperwork (ask your coach)

In Case of Injury

  1. Inform an athletic trainer or your coach about the injury.
  2. If indicated, medical referral will be made.
  3. Follow recommendations from the attending MD.
  4. Upon clearance from the athletic trainer and/or MD, you may return to competitive status.

Insurance Procedures

If a student-athlete sustains an athletic-related injury while practicing or participating in a Chabot College event, he/she must report this to the athletic training staff immediately.

  1. A referral may be made, depending on nature and type of injury suspected.
  2. Referring physician must be part of student-athletes insurance plan.
  3. If student-athlete obtains care without a referral, they will be responsible for all costs.
  4. If you are a member of an HMO, you must gain pre-authorization from your primary care provider prior to making any other appointments. If you do not gain this authorization, Chabot College will not be responsible for payment of denied claims.
  5. Chabot College's insurance coverage is a secondary provider, with limitations and exclusions.
  6. If you do not currently have medical insurance, Chabot's insurance will serve as the primary provider.


The student-athlete must take responsibility for all insurance matters. You must communicate with the training staff, insurance companies, primary care and/or team physician to ensure all procedures are followed correctly. You are ultimately financially responsible for all charges following an athletic injury. The athletic department does not assume responsibility for your medical bills.
