2021-2022 CA Dream Act Forms

Only print and submit required forms listed on your CLASS-Web account.

If you were not selected for verification, there were no messages on your Student Aid Report about conflicting information, and there are no discrepancies that must be resolved by our office, you may not have to submit any documents to our office for verification.

If eligible for aid and awarded, you will receive a message to check CLASS-Web for your financial aid offer.

Most students will have to “ACCEPT” the Terms and Conditions online prior to any funds being disbursed.

To access Financial Aid documents from this site, you need Adobe Acrobat Reader and a printer. These are not PDF-fillable forms - do not try to convert to or open with Microsoft Word or other program. You must print and complete.

If you are required to verify your Federal Tax Return information, please do one of the following:

  1. Obtain a 2019 Tax Return Transcript using the Get Transcript Online tool, or
  2. Provide a signed copy of your 2019 Tax Return (Form 1040 and attached Schedules).

Instructions for IRS Tax Return Transcript 

 

Untaxed Income Certification Form - Independent/Dependent

Certain applicants will be required to verify the untaxed inome information they provided on the CA Dream Act application. If the Financial Aid Office determines that the amounts provided and/or verified do not appear to provide sufficient financial support for the number of household members reported, the applicant and spouse/parents must explain how the family was financially supported during the 2017 calendar year.

If you were selected for this verification, if there are questions about untaxed income, or we need your certification that you are not required to file federal taxes, you will be required to complete this form. You may be required to provide proof from the IRS that you were not required to file taxes.

High School Verification Form

You must provide proof of completion of high school (High School Diploma, GED, Associate / Transfer Degree earned, or Home School)

Parent Update Form

You must provide the last name, first initial, and social security number (if they have one) for each parent or stepparent whose information is provided on your Financial Aid application. Failure to provide this information will result in a rejected financial aid application, and we cannot proceed with file review without it. Please use this form to update your parent’s information by completing all items as instructed, and have each parent sign and date form.

Student Update Form

You must provide the last name, first initial, and social security (if you have one) number. Failure to provide this information will result in a rejected financial aid application, and we cannot proceed with file review without it. Please use this form to update your information by completing all items as instructed, and sign and date the form.

Appeal for Reinstatement or Extension of Financial Aid

If you have extenuating reasons for failing to meet satisfactory academic progress standards (over 90 attempted units, or unsatisfactory performance), you must complete and submit this form to the Financial Aid Office for a Petition for Reinstatement or Continuation of Financial Aid with all required documentation.
