All faculty who wish to teach a distance education course must complete the following form and submit it to the DE chair. Note that this is the same form as posted at the Curriculum Committee/Forms webpage.

Step 3: Print your completed form, secure the required
signatures, and send both an electronic version via email and a hard
copy with the required signatures via campus mail to the
DE chair.
If you plan to use Blackboard in your Distance Education course, you'll note that the Proposal form indicates you must meet with the Instructional Designer to review/complete the Formative Evaluation Checklist. This checklist is intended as a guide; not every item will apply to your course. Use of this checklist begins with your initial consultation with the Instructional Designer (Step 8 of the Proposal Form) and continues throughout your course development until you meet with the Instructional Designer at least two weeks prior to the start of your course to complete the formative evaluation. A signed copy of the checklist must be submitted to the Committee about two weeks prior to the start of the course.
If you're assigned to evaluate an online course, the official observation form must be completed, reviewed with the evaluee, signed by both you and the evaluee, and then submitted to the appropriate Dean.
