EOPS
APPLY
- Step 1:
- Complete a Chabot College Application for Admission and return it to the Office of Admissions & Records Office.
- Complete both the mathematics and writing assessment tests. (Contact the Testing Center in Room1840, or call (510) 723-6722 for a schedule of testing dates.)
- Complete an "EOPS Sponsorship Application" and return it to the EOPS Office along with the following documents:
- Income verification:
- A copy of the previous year's Federal Income Tax Return (1040EZ, 1040A, or 1040) or
- A copy of the previous year's W2 Form(s) or
- An "Agency Certification-Untaxed Income" form available from the Financial Aid Office. Use this form to verify any non-taxable income that you may have received during the previous year or
- A "Student's (Spouse) Income Certification" available from the Financial Aid Office. Use this form to verify how you supported yourself during the previous calendar year if you did not file a federal or state income form and you did not receive untaxed income (e.g., TANF/AFDC, SSI).
- High School Transcripts: Submit an unofficial copy of your high school transcripts. Note that this is only required if you have graduated from a high school in the United States within the last year.
- College Transcripts: Submit an unofficial copy of your transcripts for each college you have attended whether or not you earned a degree or certificate.
- Step 2: Applications for financial aid -- Financial Aid forms are available through the EOPS Office or can be obtained from the Chabot College Financial Aid Office, Room 110.
- Step 3: You will be notified by mail (usually within 10 working days) of your EOPS eligibility. Qualified students will then be assigned an EOPS Counselor and are required to make an appointment with their assigned counselor.
- Step 4: At your initial appointment with your EOPS Counselor, you will be asked to complete your EOPS Student Participant Agreement (SPA) and your counselor will begin working with you to complete your Student Educational Plan (SEP).
